Case
Management: (kas manij-mint) n. a collaborative
process which assesses, plans, implements, coordinates, monitors,
and evaluates the options and services required to meet an
individuals health needs, using communications and available
resources to promote quality, cost-effective outcomes.
We
are located in Albuquerque, New Mexico and have about 90 members.
Our professional organization is targeted for nurses, social
workers and other health care professional case managers in
a diverse range of settings in New Mexico.
We
are a non-profit organization that has been affiliated since
March 11, 1997 with the national CMSA organization. The national
organization has more than 7,600 members with 100 affiliated
and pending chapters.
Our
chapters mission is to provide education, support and
communication to case managers in New Mexico. We accomplish
these goals in some of the following ways:
